Insurance Information

At Lynn C. Hyland, DDS, PC we will contact your insurance company to get an estimate of your benefits. This is not a guarantee of benefits or payment, your insurance company will determine this once they have received the final claim. We are not responsible for how your insurance company processes claims once they have received them. Services are rendered and charged to the patient/insured, not the insurance company. Be familiar with your benefits, waiting periods, exclusions, and provider participation. Uninsured patients are expected to pay their fees at the date of service. Insured patients are expected to pay any copay/co-insurance, deductible, or over maximum amounts at the date of service.

Teeth extractions are usually billed under your dental insurance.

We are in-network with most dental insurances, with the exception of MetLife and Ameritas.

We are out of network for Metlife dental, Ameritas dental and all medical insurances.

• We are out-of-network for ALL medical insurances. We do not accept BCN medical insurance.
• We do not participate with managed care dental plans; HMOs or DMOs.
• We do not take straight medicaid and we do not participate with state dental plans – such as; Healthy Kids Dental and Healthy Michigan Plan.
