After you have completed the form, please make sure to press the Complete and Send button at the bottom to automatically send us your information. Creating continuity of care care with our referral collaborator portal is easy.  Each one of your doctors can have an account as well as an office account.  Please contact our office and provide your email addresses so that we can send an invite for your office and or doctors.  Your doctor should have a unique email address other than your office email address.  Once you have an account, you can easily refer patients with all your information pre-populated, view shared images for treatment planning, and so much more.
